What You'll Be Doing

As you complete this form, you'll be asked a series of questions about the grocery store you are assessing. These may include:

Availability of fresh produce Quality of meat and dairy products Pricing and discounts Accessibility features (e.g., ramps, wide aisles) Presence of culturally diverse food options Overall cleanliness and customer service Time Commitment

This assessment is expected to take approximately 20-30 minutes of your time. Feel free to take notes or photos as you move through the store to assist you in filling out the form accurately.
